Sepsis-associated encephalopathy as a differential diagnosis with motor deficit plus altered mental status.

چکیده
Sepsis-associated encephalopathy (SAE) is a poorly understood and underdiagnosed neurological complication of sepsis. Several conditions commonly associated with sepsis (hepatic or renal dysfunction, electrolyte abnormalities, acid-base disturbances, glycemia disturbances, hypotension, hypoxemia, sedation, body temperature disturbances, and neuroendocrine dysfunction) make SAE evaluation difficult. Sepsis-associated encephalopathy has been reported to occur in 8% to 70% of septic patients. This large range derives from the different definitions of sepsis and encephalopathy used in the current literature. In some cases, SAE may precede other manifestations of sepsis. We report a case of a 37-year-old pregnant woman who developed septic encephalopathy for whom the initial symptom was confusion that evolved with motor deficit and subsequently to coma.

Nonconvulsive Status Epilepticus in Hepatic Encephalopathy
We discuss a case of a 64-year-old male with a history of liver failure presenting with altered mental status, initially diagnosed with hepatic encephalopathy but ultimately diagnosed with nonconvulsive status epilepticus (NCSE) by electroencephalogram (EEG). NCSE is a difficult diagnosis to make, given no clear consensus on diagnostic criteria. Especially in the intensive care unit setting of ...
متن کاملHypoxic-ischemic encephalopathy in a young man due to tramadol overdose
Objective: Tramadol is a synthetic analgesic with two mechanisms. The opioid and non-opioid mechanisms are responsible for tramadol side effects. Non-opioid side effects of tramadol are due to the reuptake inhibitions of serotonin and norepinephrine. Some of the side effects include anaphylactoid reactions, CNS depression, hypoglycemia, hypotension, respiratory depression, seizures, and seroton...
متن کاملDiabetic Encephalopathy Affects Mitochondria and Axonal Transport Proteins
Introduction: Diabetic encephalopathy is described as any cognitive and memory impairments and associated with hippocampal degenerative changes, include neurodegenerative process and decreased number of living cell. Mitochondrial Diabetes (MD) appears fallowing activation of mutant mitochondrial DNA and is combination of diabetes and cognitive deficit. In this research we showed the correlation...
متن کاملOlanzapine Overdose in a Pin Point Pupil with Altered Sensorium
Background:Olanzapine is a highly tolerable and easily affordable atypical antipsychotic drug which has been commonly prescribed in both inpatient and outpatient settings for several mental disorders. Olanzapine overdose is commonly seen in psychiatric patients, who attempt suicide by intoxicating themselves with their own prescribed medications. Increased olanzapine use is associated with incr...
متن کاملCranial neuropathies, confusion, and ataxia— Challenges for diagnosis and therapy
Introduction: The differential diagnosis of patients presenting with multiple cranial neuropathies, ataxia, and altered mentation is broad and includes immunologic, infectious, vasculitic and metabolic conditions. Primary considerations are Bickerstaff’s brainstem encephalitis (BBE), the Miller Fisher syndrome (MFS), Wernicke’s encephalopathy and botulism. The initial workup may be unrevealing....
